Discharge from hospital: support for elderly patients

Patient discharged from hospital

For most patients, returning home after a hospital discharge is a relief. However, some may feel anxious if they live alone or need after-care support.

For hospitals and health professionals, a discharge plan means the patient is able to return home sooner and have confidence they can continue their recovery safely.

Whilst there is no standard NHS discharge process, a monitored personal alarm can provide effective home care support.

For patients who are having medical treatment, discounted rates or specialist packages may be available

Get in touch by completing the form below and one of our team will evaluate your patient’s needs and recommend a suitable personal alarm.

If you would like leaflets to share with your patients, then you can request these too.

How a personal alarm can support hospital patients

Patients may be at increased risk of a fall after hospital treatment due to increased fragility or changes in medication. A personal alarm provides reassurance that help is available.

All our in-home personal alarms include:

  • A life-saving alarm pendant and base unit.
  • 24-hour monitoring so they can get help fast in an emergency, any time of the day or night.

A personal alarm with fall detection can automatically alert our Emergency Resolution Team when a fall is detected without the patient needing to press their emergency pendant button.

Medical alert protection systems can reduce visits to the emergency department and reduce the length of stay in hospital if admission by almost half, from 15 days to 8 days.

A very good device for those who live alone and have no-one on hand if one has a fall and needs medical help. It saved my life when I had an accident in 2016. I cannot contemplate life without it.
