We just need a few details so we can respond quickly in an emergency.
The form takes around 3 -5 minutes to complete, and it must be finished in one go, so please have the information below ready before you start.
Important: Any details you enter will replace the information we currently hold.
You can complete this on behalf of the alarm user.
The information you provide will be used to update your emergency contact list. It is your responsibility to ensure that all details entered are accurate and complete. Incorrect or missing information may affect our ability to respond appropriately in an emergency.
What you’ll need
1. Details of the alarm user
- Full name
- Date of birth
- Phone number and email address (if they have one)
- Postcode
- Relevant medical information (including NHS number and allergies, if known)
2. Emergency contacts (up to 4)
For each contact:
- Full name and address
- Phone number and email address
- Their relationship to the alarm user
- Whether they are a keyholder or can access a key safe
