Autumn from Taking Care
A non-clinical wellbeing service.

Autumn is a personalised, non-clinical wellbeing service designed to help support people to stay active, confident and independent. Autumn learns what’s normal for each person and identifies meaningful changes early before small issues become bigger problems.

    Identifying meaningful change earlier

    Many people experience a gradual decline in activity, mobility and confidence long before a crisis occurs. Too often support begins only after needs have escalated and become more serious and costly.

    Autumn helps identify meaningful changes in adults 65 + living independently, particularly those at risk of early frailty, falls or functional decline. Autumn learns what is normal for these individuals, intervenes earlier and helps delay or prevent escalation into formal care.

    What the service includes

    Using a simple activity tracker and mobile app, Autumn is designed for people 65 + older who are still independent who would benefit from a light touch prevention support. Autumn encourages them to maintain activity, confidence and routine for longer.
